Phase 2 of Essex County's OPHM (Office of Public Health Management) Feasibility Study. Your Input Matters!

Dear Colleagues:

 Thank you for your continued investment in the Essex County Access to Care Feasibility Study. We are now entering Phase 2 of our study and are inviting you to participate in a rating activity. Through this activity, we seek your input on the importance and feasibility of the ideas identified during the earlier brainstorming activity. This activity is crucial for identifying priorities and developing actionable plans to achieve them.

For the rating activity, we ask you to provide your opinion about the ideas collected and assign values to each based on their importance and feasibility. Additionally, you will be asked to complete a short series of background questions about your role in the Essex County community or health profession. The rating process and demographic questions should take approximately 25-35 minutes.

We plan to keep the survey open until two weeks from today. Your responses will be automatically saved, and you can return to the survey at any time while it is open to complete or edit your responses.
